QUESTION:
     I have heard that there is a new medication for Migraines. I usually just take a Maxalt when the headache starts. But that makes me so tired that I can barely function. The problem is that I usually wake up with a migraine. If I get tired, that I have a problem working, feel like a zombie.
     Can you tell me the name of this new medication. I would like to ask my Doctor if it might help me. I believe it is a maintenance, and have also heard that rapid weight loss is a side affect. Could extra weight also be causing these migraines. I would love to be rid of my headaches, but losing a few unwanted pounds would also be welcoming.

Dear cpc,
    Since you mention “maintenance” and “rapid weight loss,” I think you’re referring to Topamax (topiramate), which is a neuronal stabilizing agent. It was originally developed for seizure disorders, and is now in clinical trials as a Migraine preventive.
    It should be noted that the weight loss associated with Topamax is a potential side effect, not experienced by all patients. Also, in trials so far, it seems to be directly proportional to the weight of the patient, with those experiencing this side effect losing an average of 3.8% of their beginning body weight.
    I know of no direct correlation between extra weight and Migraine disease. Certainly, those with many Migraines tend to be less physically active, and that can present a problem. Migraines are not just headaches, but a neurological process. Migraine is a genetic neurological disease, and the headache that usually accompanies a Migraine attack is ONE symptom.
    Bottom line: If you’re experiencing three or more Migraine attacks per month, you’re a good candidate for preventive medications and should consult your doctor. However, I’d discuss all options with him, and not elect to try Topamax only because of the potential side effect of weight loss.

The tiredness after Maxalt is probably the postdrome phase, since it works to remove the headache. Even though it's a Migraine abortive, you may still experience after the pain subsides.
