Question: ...the difference between the Chiari malformation headaches and the migraines?

Answer:

Dear Anita:
I really feel for your different problems. People with Chiari malformations can often have headaches, and they can have migraines as well. If you get two different types of headaches, both can be treated. The migraines are probably easier to treat with specific medications, such as the triptans. Chiari type headaches often involve elevated fluid pressure in the brain; they can be related to body position or to lifting, straining, etc. These are tougher to treat successfully. Sounds like Topamax might be adding to problems. There are many other medications for suppressing migraines. Many of these are in the same general family as Topamax. Getting to a headache specialist would be key for best management of your complicated headache pattern.
